STEP 3: Request an autographed photo. Fill out the form below then send your SASE (self-addressed, stamped envelope) to the fan club address. If you skip this step if you will not receive an autographed photo.

Please make sure your SASE is large enough for an 8x10. To calculate postage costs, visit the USPS web site. Use Chicago zip code "60680" when figuring the postage price.

Please allow 6 to 8 weeks for your request to be processed. International requests may take longer. The limit is one photo per e-mail address.
